To Enter Additional Staff:
Fill in the LAST NAME, FIRST NAME, and MIDDLE INITIAL
Fill in the USER NAME (USERID) The user name is the first 7 characters of the last name
and the first initial of the first name. If you receive an error message that this userid is
already in the system when you attempt to save the record, you will need to add a
number at the end of the user name (start with 1 - if that's there, use2, etc.) to create a
unique userid. For example, if the user's name is Pat Jefferson, the user id would be
jeffersp. If jeffersp is already in the system, try jeffersp1. If that is already there, try
jeffersp2 and so on.
Fill in the PASSWORD, which will be tempmwa## (Use fia, foc, etc. as appropriate
instead of the mwa. Replace ## with your MWA number.)
Fill in the JOB TITLE, PHONE NUMBER (###-###-####), AND EMAIL ADDRESS.
Next, select your MWA, the SERVICE CODE, and PRIMARY LOCATION from the
drop down lists.
Note: The Provider and location Information must already be in the system before you add staff. If not, you will need to send and email to Implementation@esa.state.mi.us with the name, address, phone number,
county, MWA of the Provider and/or Location to be added.
Select the STAFF MEMBER JOB TYPE (such as MWA Case manager, etc.).
The default status is ACTIVE. It can be changed to inactive or pending, but is normally
not changed.
When done, click on INSERT to save the record.
